The Gardens, Resident Care Apartment Complex, is certified through the WI Department of Health and Family Services. This 26 unit apartment complex surrounds a spectacular, three floor atrium. These apartments are designed for residents who still prefer the lifestyle of independence, but may desire some special services from time-to-time. Efficiency, one bedroom or two bedroom apartments are included in this complex.

Individuals residing at the Gardens are able to receive up to 28 hours per week of care and services. This may include the following:

• 24-hour Staff Assistance
• Overall Health Monitoring by Registered Nurse
• Medication Management
• Meal Plan Options
• Social and Therapeutic Activity Programs
• Personalized Pendant/Voice Response Emergency Call System
• Housekeeping/Laundry
• Utilities

Assisted Living at Stone Crest Residence

Stone Crest Residence, a Community-Based Residential Facility located on the Forest Park Village Campus, is licensed through the Department of Health and Family Services. This facility accommodates 16 men or women in need of supportive services, while promoting independence. Accommodations are private rooms with own half bathroom.

Individuals residing at Stone Crest Residence are able to receive up to 3 hours of skilled care per week and supportive services to meet all activities of daily living.
This includes:

• 24-hour Staff Assistance
• Overall Health Monitoring by Registered Nurse
• Medication Management
• Meals and Snacks Planned by a Registered Dietician
• Social and Therapeutic Activity Programs
• Housekeeping/Laundry
• Scheduling of Physician Appointments
