Independent Living
Interested individual(s) need to complete an Application for Residency in order to be considered a resident of Forest Park Village. After the form is completed, the Director of Operations will review the application, validate references, complete a credit report and conduct a personal interview.
After the application is approved and an apartment selected, then one month's rent is due for the Security Deposit, and a one-year lease should be signed.

Assisted Living
The Gardens
Interested individual(s) need to complete an Application for Residency in order to be considered a resident of Forest Park Village. After the form is completed, the Director of Operations will review the application, validate references, complete a credit report and conduct a personal interview. In addition, a Health Assessment needs to be completed by the Registered Nurse.
After the application is approved and the Health Assessment completed, then a Security Deposit is due and the Resident Agreement should be signed.

Assisted Living
Stone Crest Residence
Interested individual(s) must be over 62 years of age and free of communicable diseases. A physical exam and TB skin test performed by a physician needs to completed prior to move in. In addition, a Health Assessment needs to be completed by the Regestered Nurse or Facility Program Manager.
